Instructions on how to fill out new patient and other forms.
Please fill out as completely as possible. We understand there are many pages to fill out, however the more thorough you are, the better the assessment will be:

Select the appropriate form for your specific age group or situation
Step 2
You now have 2 options once you have selected the correct form.
- Option 1 - Print the form, fill it out manually, and bring it with you to your appointment. To print this form select the "print form" button located on the top right hand corner of the form.
- Option 2 - Fill the form out online. Place the cursor on the correct line that you want to fill out and begin typing your information. There are 2 options from this point.
1.) Select the "print form" button and bring the paper work to your appointment 2.) Select the "submit by e-mail" button and your paper work will be e-mailed directly to our office - If you have Microsoft Outlook Express, select the "submit by e-mail" button and hit send. - If you receive a message that says "Select Email Client" either choose Desktop Email Application (Microsoft Outlook Express) or Internet Email. - If you use an Internet e-mail service such as Yahoo or Hotmail you will need to save the New Patient PDF Forms to your desktop (if you would like a copy of your forms, hit the "print button" at this time). - Click File, Save As, then save the form onto your Desktop. - Then open your web-based e-mail service and create a new message. - You will need to attach the New Patient Form into your e-mail. - Once you have attached the forms you can send the e-mail to: redrockchiro@hotmail.com. * Please print and fill out manually the MSHSL sports physical and DOT/CDL physical forms and bring them into our office.
