We accept most insurance plans, and our staff will be glad to review your benefits with you before your visit, free of charge. Of course, we welcome patients who don't have insurance, and do our best to keep our fees as reasonable as possible so that no one is limited in the care they receive. Instructions on how to fill out insurance/payment forms. Please fill out as completely as possible:
Step 1
Select the appropriate form for your specific age group or situation
- Payment policy
- Aflac form
- Combined INS form
Step 2
You now have 2 options once you have selected the correct form.
- Option 1 - Print the form, fill it out manually, and bring it with you to your appointment. To print this form select the "print form" button located on the top right hand corner of the form.
- Option 2 - Fill the form out online. Place the cursor on the correct line that you want to fill out and begin typing your information. There are 2 options from this point.
- Select the "print form" button and bring the paper work to your appointment - Select the "submit by e-mail" button and your paper work will be e-mailed directly to our office
